How do you write your books?

Seriously, how do you sit down and write a book? I'm considering writing a book of my own, but I'm really not sure how to really get started. Do you tend to outline everything (and how detailed of an outline do you use if you utilize that approach) and then start writing, or do you make things up as you go?

On another response in these ATA questions, I noticed that you used a name generator. Which one do you use? I'm having difficulty coming up with names, and at this point I'm just blatantly stealing names from relatively obscure historical figures.

Finally, how much do you tend to write in a day/what constitutes a good day's worth of work for you when writing a book?
